This is a sample application to become a Supplemental
Educational Services (SES) provider. Please be aware that SES applications
vary from state to state, but this
proposal from the
state of Minnesota may give you an idea of what it takes to become an
approved provider of supplemental services. When you write your proposal,
show the reviewers how your organization will help improve the academic
success of children in your area.
All applications to become an approved provider of supplemental services
are reviewed by state
departments of education.
